This serves as your application and request form for long term training. 

It also helps me for those of you who are requesting short term training or consulting.

This form is also called Main 1 and 2 Train Dog Questionnaire.

Please note, I offer fully trained turn key adult dogs, these forms are used for purchasing training or trained dogs.  I will contact you separately to follow up with your training request.

The self screening for main 1 is required for all persons interested in purchasing a trained Service Dog, general purpose any elective.
