DTC Trained Dog Questionnaire required for ALL

 Online / In person / Hybrid handlers in training with Crystal or Ed. 

This serves as your application and request form for long term training. 

It also helps us for those of you who are requesting short term training or consulting.

This form is also called Main 1 and 2 Train Dog Questionnaire.

Step 3: after I receive your request for training I will ask you to email me your questionnaire.

Step 4: mobility assistance dog teams must utilize self screen for main 1 to determine if you are ready to begin training with your dog with Crystal.

General purpose service dog and handler team training program turns out qualified dog teams as:

• Mobility assistance
• Therapy/wellness dogs
• Emotional support/psychiatric
• Hearing: partial deafness handler assist

Main 2 AFD Use Main 2 Questionnaire 

Athletic family dog and handler training program turns out qualified dogs teams as:

• Fitness partner gym and outdoor
• Urban pet
• Outdoor hiking partner
• Obedient family pet
• Practical Protection by Ed Marshall
